Abstract

Two emm 81 throat and blood streptococcus pyogenes isolates were obtained 13 days apart from an individual patient. They were genetically similar but showed multiple phenotypic differences, the most remarkable being the complete lack of streptococcal pyrogenic exotoxin S (speB) functional activity in the blood but not the throat isolate. PFGE analysis using 5 different restriction enzymes and sequencing of selected genes confirmed their genetic similarity. Immunoblots of culture supernatants established that the speB was secreted only by the throat isolate. Immunoblots of cell lysates showed that both produced the SpeB precursor but only the throat isolate processed the mature form. Real time RT-PCR established differences in speB transcription. Microarray analysis comparing the two isolates revealed many transcriptional differences suggesting that a regulatory gene, perhaps due to an epigenetic change, is responsible for the observed phenotypic differences of these two isolates.
