Abstract
The purpose of this evidence-based project was to train the Kalihi-Palama Health Center (KPHC) Care Coordinators (CCs) to use the AHRQ ‘teach back method’ to provide patient education on diabetes eye complications to help reduce the funduscopic no-show rate among patients diagnosed with Type 2 Diabetes. Project objectives included: (a) by mid to end August 2019, develop a CC protocol to implement the AHRQ ‘teach back method’ for patient education on diabetes eye complications with use of an interactive eye chart; (b) by the end of August 2019, train KPHC CCs on how to implement the CC protocol on AHRQ ‘teach back method’ for patient education on diabetes eye complications; (c) by September 2019, collect baseline information on funduscopic exam ‘no show rate’ at KPHC; (d) from September to October 2019, implement CC protocol using AHRQ ‘teach back method’ for all Type 2 Diabetic patients who have a clinic visit at KPHC; and (e) by the end of October 2019, conduct a trend analysis of the funduscopic no-show rates at KPHC, comparing baseline information to end of implementation information. The fundoscopic ‘no show rate’ at the start of the project was 53.8%. Throughout the year the fundoscopic ‘no show rate’ began to trend downward with a final fundoscopic ‘no show rate’ of 25.0% and an overall reduction of 28.8% at the end of project implementation. The findings revealed that training the CCs to use the teach-back method helped reduce the fundoscopic no-show rate among Federally Qualified Healthcare (FQHC) patients diagnosed with Type 2 Diabetes and improved CCs conviction and confidence in using the ‘teach-back method.’
